You probably need to remove the dust cap and the cotter pin to tighten the axel shaft nut. My wife's 2004 dune 150 gets loose all the time due to wear, tighten it and use a new cotter pin.
I like to use a half inch ratchet and get it close to the next hole for the cotter pin. But sometimes it is to far to the next hole without over tightening it so I have used different thickness washers or crossed the axel nuts left to right to get it tight without over tightening it and stripping it out. Or swapped the axel washers or flip the axel washers.

I am going to replace the hubs on my wife's buggy some day but if you keep the axels tight it will keep on keeping on.
